New Patient Forms
In December 2016, ReturningSpring® switched to all electronic healthcare records and scheduling using the HIPAA-compliant UnifiedPractice™ EHR. This allows us to safely store and access your data, provide for convenient 24/7 on-line scheduling, and help us work with your insurance companies. Most importantly, it helps us work towards one of our long-term environmental goals — a paperless office.
When you schedule your first appointment, our system will send you a link by email which will allow you to fill out (and sign) your paperwork online in our secure system. If you prefer, you may also complete the forms on an i-Pad at our office. Just let us know!
